The form must be accompanied by a <strong>legalized or notarized copy of the valid identification document<\/strong> of the person signing. Only citizens of European Union member states may self-certify documentation, where applicable.<\/p>\n<p>For minor applicants (under 18 years of age) the parental consent form must be signed by both parents, unless a court-issued divorce order grants sole decision-making responsibility to one parent; only one parent is surviving; or a court-appointed legal guardian is authorized to sign on behalf of the minor.<\/p>\n<p><strong>Please note:<\/strong>\u00a0If the financial guarantor, or one or both parents of a minor applicant (as applicable), reside <strong>outside this Consulate\u2019s jurisdiction<\/strong>, the signature on the relevant affidavit or consent form must be authenticated as follows:<\/p>\n<ol type=\"A\">\n<li><strong> Apostille<br \/>\n<\/strong>If the country of residence is a signatory to the <a href=\"https:\/\/www.hcch.net\/en\/instruments\/conventions\/specialised-sections\/apostille\"><strong>Hague Apostille Convention<\/strong><\/a>, the document must be <strong><em>apostilled<\/em><\/strong> in that country. The Apostille Convention simplifies the authentication of public documents for use in more than 125 member countries, including for example: China, Brazil, Mexico, India, Japan, Philippines, United States.<br \/>\n<strong>OR<\/strong><\/li>\n<li><strong>Consular Legalization<br \/>\n<\/strong>If the country of residence is not a signatory to the Apostille Convention (for example, Iran), the document must be <a href=\"https:\/\/www.esteri.it\/en\/servizi-consolari-e-visti\/italiani-all-estero\/traduzione-e-legalizzazione-documenti\/\"><strong><em>legalized (notarized)<\/em><\/strong><\/a> by the Italian Consulate responsible for the guarantor\u2019s or parent\u2019s place of residence.
